Sabine & Wellwood Family Medicine provides compassionate, patient-centred care as part of the Wolfville Collaborative Family Practice. Dr. Wellwood and Dr. Sabine work alongside a multidisciplinary team — six family doctors, two family practice nurses, dietitians, a social worker, an occupational therapist, and a certified exercise physiologist — to deliver comprehensive care for individuals and families in our community.
Our Approach
We believe good healthcare goes beyond diagnosing and treating illness. Our goal is to help you stay well through preventive screenings, immunizations, and personalized lifestyle guidance. We provide primary care across all ages and stages of life, using evidence-based practices in a caring, confidential, and non-judgmental setting.
Collaborative Care
As part of a collaborative practice, you benefit from the expertise of multiple healthcare professionals working together. This team-based approach means you’re always seen by the most appropriate provider for your needs, resulting in more timely, coordinated care.
Depending on your health concerns, different members of our team may be involved in your care:
- Dietitians and our Kinesiologist can support you with nutrition and physical health strategies.
- Family Practice Nurses may follow up on test results or help manage ongoing health needs.
- Our Social Worker is available for support with emotional well-being and life challenges.
- Our Occupational Therapist can help with functional challenges affecting your daily activities and independence.
By working together, we can address the full range of your health needs and make it easier to access allied health services such as physiotherapy, massage therapy, chiropractic care, and mental health counselling.
